Онкологические заболевания и химиотерапия (ХТ) провоцируют развитие эмоциональных и когнитивных нарушений, что значительно ухудшает качество жизни пациентов и снижает приверженность лечению. Около трети пациентов испытывают высокий уровень дистресса, который часто остается недиагностированным. Химиоиндуцированная полиневропатия (ХИПН) усугубляет психическое состояние пациентов, а эмоциональный дистресс, в свою очередь, обостряет восприятие симптомов.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: определение взаимосвязи между невропатическими, эмоциональными и когнитивными нарушениями у пациентов, получающих нейротоксическую химиотерапию.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включен 71 пациент со злокачественными новообразованиями, получавшими полихимиотерапию на основе платины или таксанов. ХИПН диагностирована у 73% пациентов. Выраженность астении, определявшейся с использованием многомерного опросника оценки утомления MFI-20 (Multidimensional Fatigue Inventory), депрессии и тревоги (шкала Гамильтона), а также когнитивных функций — по тесту MoCA (Montreal Cognitive Assessment), степень нейропатии по шкале неврологического дефицита (ШНД) (Neuropathy Disability Score (NDS)), оценивались до и после ХТ.</p></sec><sec><title>Результаты</title><p>Результаты. После ХТ зафиксировано статистически значимое нарастание астении (общая астения: с 9 до 12 баллов (p = 0,017). Наличие депрессии увеличилось с 28 до 49% пациентов, а тревога оставалась стабильно высокой — (у 79 — 81%) пациентов. Женщины демонстрировали более выраженные нарушения (астения, депрессия и когнитивный дефицит). Наиболее тяжелые эмоциональные расстройства имели связь со степенью выраженности ХИПН.</p></sec><sec><title>Заключение</title><p>Заключение. ХТ усугубляет эмоциональные и когнитивные нарушения, особенно у пациентов с невропатией, что требует комплексного наблюдения и включения психологической поддержки в лечебный процесс.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cancer and its treatment with chemotherapy (CT) can induce or exacerbate emotional disturbances and cognitive impairment, significantly deteriorating patients’ quality of life and reducing treatment adherence. Approximately one-third of patients experience a high level of distress, including anxiety and depression, which often remains undiagnosed. Chemotherapy-induced peripheral neuropathy (CIPN) worsens the psychological state, increasing anxiety and depression, while emotional distress, in turn, heightens the perception of symptoms. These interrelated disorders necessitate a more attentive approach to their diagnosis and management.</p></sec><sec><title>Objective</title><p>Objective: to determine the relationship between neuropathic, emotional, and cognitive disorders in patients who have received neurotoxic chemotherapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 71 patients with solid malignant tumors receiving platinum — or taxane — based polychemotherapy. CIPN was diagnosed in 73% of patients. Levels of asthenia (MFI-20), depression and anxiety (Hamilton Rating Scale), cognitive functions (Montreal Cognitive Assessment (MoCA)), and neuropathy severity (Neuropathy Disability Score (NDS)) were assessed before and after CT.</p></sec><sec><title>Results</title><p>Results. Following CT, a statistically significant increase in asthenia was recorded (general fatigue: from 9 to 12 points (p = 0.017). The prevalence of depression increased from 28 to 49% while anxiety remained consistently high (in 79 — 81% of patients). Female patients demonstrated more pronounced asthenia, depression, and cognitive deficits. The most severe emotional disorders were dependent on the severity of CIPN.</p></sec><sec><title>Conclusion</title><p>Conclusion.
