Целью работы является оценка эффектов когнитивной реабилитации после коронарного шунтирования (КШ) с помощью электроэнцефалографических (ЭЭГ) показателей у пациентов в зависимости от наличия гемодинамически незначимых стенозов внутренних сонных артерий (ВСА).</p><sec><title>Материал и методы</title><p>Материал и методы. В когортном проспективном исследовании приняли участие 85 пациентов (16 женщин, 69 мужчин), в возрасте от 45 до 75 лет, поступившие на плановое КШ. Всем пациентам проведено клинико-инструментальное, неврологическое, расширенное нейропсихологическое и ЭЭГ-обследование.</p></sec><sec><title>Результаты</title><p>Результаты. После проведения многозадачного тренинга у пациентов со стенозами КА были выше показатели спектральной мощности биопотенциалов высокочастотных бета1 и бета 2 ритмов (13-30 Гц), тогда как спектральная мощность альфа1- ритма (8-10 Гц) у этой группы была ниже по сравнению с пациентами без стенозов. В случае успешной когнитивной реабилитации (при отсутствии послеоперационной когнитивной дисфункции (ПОКД)) на 11-12-е сутки после КШ по сравнению с группой с ПОКД пациенты без стенозов имели более высокие показатели суммарной мощности биопотенциалов альфа1- ритма и более низкие — бета-ритмов, чем пациенты со стенозами ВСА.</p></sec><sec><title>Заключение</title><p>Заключение. Отсутствие стеноза в каротидном бассейне в случае успешного прохождения многозадачного тренинга способствовало оптимизации мозговой активности с увеличением мощности биопотенциалов альфа-ритма. Тогда как пациенты с наличием стенозов КА как в случае успеха многозадачного тренинга в раннем послеоперационном периоде КШ, так и при его отсутствии характеризовались увеличением бета-активности. Данное исследование может являться обоснованием для разработки персонифицированных программ когнитивной реабилитации у пациентов с со стенозами ВСА.</p></sec></abstract><trans-abstract xml:lang="en"><p>The effect of atherosclerotic lesions of the carotid arteries (CA) on the success of the cognitive function recovery procedures in cardiac patients has not been sufficiently studied. This study aims to evaluate the effects of cognitive rehabilitation depending on the presence of CA stenosis in patients after coronary artery bypass grafting (CABG) using neurophysiological indicators.</p><sec><title>Material and methods</title><p>Material and methods. The cohort prospective study involved 85 patients (16 of them women), aged 45 to 75 years, admitted for elective CABG. All patients underwent standard clinical and instrumental, neurological, extended neuropsychological and neurophysiological examination.</p></sec><sec><title>Results</title><p>Results. After multi-task training, the patients with CA stenosis had higher levels of high-frequency beta power (13-30 Hz), while the spectral power of the alpha1 rhythm (8-10 Hz) in this group was lower compared to patients without stenosis. In the case of successful cognitive rehabilitation (absence of POCD) at 11–12 days after CABG, compared with the group with POCD, patients without stenoses had higher total power of alpha 1 power and lower — beta power than patients with stenoses CA.</p></sec><sec><title>Conclusion</title><p>Conclusion. The absence of CA stenosis in the patients with successful completion of multi-tasking training contributed to the optimization of brain activity with an increase of alpha power. Whereas the patients with CA stenosis were characterized by an increase in high-frequency brain activity, both in the case of successful multi-tasking training and in its absence, in the early postoperative period of CABG.
