Поверхностный сидероз центральной нервной системы — хроническое прогрессирующее заболевание, обусловленное субпиальным отложением продуктов распада гемоглобина вследствие длительного истечения малых объемов крови в субарахноидальное пространство. При инфратенториальном поверхностном сидерозе (ИПС) отложение гемосидерина происходит преимущественно на поверхности мозжечка, ствола мозга и спинного мозга. Ранняя диагностика ИПС способствует повышению эффективности лечения и предотвращению тяжелой инвалидизации.</p><p>   Цель исследования — анализ клинико-нейровизуализационных, инструментальных и лабораторных характеристик серии собственных наблюдений пациентов с ИПС.</p></sec><sec><title>   Материал и методы</title><p>   Материал и методы. Обследованы 7 пациентов с ИПС (4 мужчины, 3 женщины). Проведена оценка клинической картины, МРТ головного и спинного мозга, нейрофизиологических исследований, УЗИ органов брюшной полости и лабораторных показателей обмена железа.</p></sec><sec><title>   Результаты</title><p>   Результаты. Медиана возраста на момент осмотра составила 63 года, медиана длительности заболевания — 3 года. Причины развития ИПС установлены у четырех пациентов. Клиническая картина в большинстве случаев была представлена прогрессирующей мозжечковой атаксией и нейросенсорной тугоухостью. По данным МРТ головного мозга у всех пациентов были выявлены типичные для ИПС нейровизуализационные признаки.</p></sec><sec><title>   Заключение</title><p>   Заключение. С развитием методов нейровизуализации ИПС распознается все чаще, что требует от клиницистов осведомленности о данном заболевании, понимания его причин и наиболее оптимальной тактики ведения пациентов. Необходимо дальнейшее изучение методов лечения ИПС с целью редукции токсического влияния свободного железа на структуры ЦНС и уменьшения выраженности клинических проявлений заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Introduction</title><p>   Introduction. Superficial hemosiderosis of the central nervous system is a chronic, progressive disease caused by continuous low-volume bleeding into the subarachnoid space and, as a result, subpial deposition of hemoglobin de-tritus. Infratentorial superfi cial hemosiderosis (ISS) is characterized by hemosiderin accumulation primarily on the surface of the cerebellum, brainstem and spinal cord. Early detection of ISS may prevent severe disability and raise the chance of successful medical treatment.</p><p>   The purpose of this study was to analyze clinical, radiological, instrumental, and laboratory findings, based on a series of ISS cases diagnosed at the Research Center of Neurology.</p></sec><sec><title>   Material and methods</title><p>   Material and methods. Seven patients diagnosed with ISS were included in this study (4 men, 3 women). Evaluation of the clinical findings, disease history, brain and spinal cord MRI, MR-angiography, brainstem auditory evoked potentials, audiometry, abdominal ultrasound, and laboratory iron metabolism tests was performed.</p></sec><sec><title>   Results</title><p>   Results. The median age of subjects was 63 years, and the median duration of disease was 3 years. We identified the cause of ISS in four patients. The most common clinical symptoms were cerebellar ataxia and sensorineural hearing loss. All patients had a typical ISS pattern on brain MRI images.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. With the development of neuroimaging, iSS is becoming a more frequent finding. Clinicians should be aware of its causes and optimal management strategy.
