Воспалительные изменения в стенке сосудов играют важную роль в прогрессировании атеросклероза сосудов головного мозга и возникновении его осложнений, в том числе инсульта. Влияние транскраниальной микрополяризации и методов вторичной профилактики инсульта на развивающиеся в остром периоде острой мозговой катастрофы воспалительные изменения крови до конца не изучено.</p></sec><sec><title>   Цель исследования</title><p>   Цель исследования: оценить изменение показателей крови, отражающих воспалительные изменения, у пациентов в раннем восстановительном периоде ишемического инсульта на фоне комплексных реабилитационных мероприятий, включающих транскраниальную микрополяризацию и меры вторичной профилактики.</p></sec><sec><title>   Материал и методы</title><p>   Материал и методы. Обследовано 78 пациентов в раннем восстановительном периоде ишемического инсульта в возрасте от 42 до 75 лет (средний возраст 59,50 [54,50; 64,50]). Пациенты разделены на 2 группы. В 1-й группе применялась транскраниальная микрополяризация (46 чел., 59,0 %), во 2-й — не применялась (32 чел., 41,0 %). Всем пациентам перед началом курса реабилитации и после его завершения определялось содержание в крови С-реактивного белка, фибриногена, гликопротеина sCD40L, гомоцистеина и мочевой кислоты.</p></sec><sec><title>   Результаты</title><p>   Результаты. Уровень С-реактивного белка, фибриногена и мочевой кислоты у пациентов за курс реабилитации существенно не менялся, а уровень гликопротеина sCD40L превышал нормативные значения до и после курса, снижаясь в ходе лечения у больных 1-й группы (p = 0,027). Концентрация гомоцистеина крови на протяжении курса реабилитации находилась в пределах референсных показателей, уменьшаясь у пациентов 1-й группы (p = 0,004). Выявлена дозозависимая сопряженность между приемом аторвастатина и снижением уровней мочевой кислоты, С-реактивного белка и фибриногена.</p></sec><sec><title>   Заключение</title><p>   Заключение. Курс реабилитации с применением транскраниальной микрополяризации у пациентов в раннем восстановительном периоде инсульта сопровождается снижением уровня гликопротеина sCD40L и гомоцистеина крови. Агрессивная терапия статинами сопровождается улучшением показателей С-реактивного белка, фибриногена и мочевой кислоты крови.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Background</title><p>   Background. Inflammatory changes in the vascular wall play an important role in the progression of cerebral atherosclerosis and the occurrence of its complications, including stroke. The infl uence of transcranial direct current stimulation and methods of secondary prevention of stroke on the inflammatory changes in the blood that develop in the acute period of acute cerebral catastrophe has not been fully studied.</p></sec><sec><title>   Objective</title><p>   Objective: to evaluate changes in blood parameters reflecting inflammatory changes in patients in the early recovery period of ischemic stroke against the background of complex rehabilitation measures, including transcranial direct current stimulation and secondary prevention measures.</p></sec><sec><title>   Material and methods</title><p>   Material and methods. 78 patients in the early recovery period of ischemic stroke aged from 42 to 75 years (average age 59.50 [54.50; 64.50]) were examined. Patients are divided into 2 groups: 1 — transcranial direct current stimulation was used (46 people, 59.0 %), 2 — it was not used (32 people, 41.0 %). In all patients, before starting the rehabilitation course and after its completion, the blood levels of C-reactive protein, fibrinogen, glycoprotein sCD40L, homocysteine and uric acid were determined.</p></sec><sec><title>   Results</title><p>   Results. The level of C-reactive protein, fibrinogen and uric acid in patients during the rehabilitation course did not change signifi cantly, and the glycoprotein sCD40L exceeded the normative values before and after the course, decreasing during treatment in patients of group 1 (p = 0.027). The blood homocysteine concentration during the rehabilitation course was within the reference values, decreasing in patients of group 1 (p = 0.004). A statistical analysis revealed a dose-dependent relationship between taking atorvastatin and a decrease in the levels of uric acid, C-reactive protein and fibrinogen.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. A course of rehabilitation using transcranial direct current stimulation in patients in the early recovery period of stroke is accompanied by a decrease in the level of glycoprotein sCD40L and blood homocysteine.
