Церебральная аутосомно-доминантная артериопатия с субкортикальными инфарктами и лейкоэнцефалопатией (ЦАДАСИЛ) — один из распространенных вариантов болезни малых сосудов. Определение нейровизуализационных маркеров актуально для своевременной диагностики ЦАДАСИЛ.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Определение потенциальных маркеров ЦАДАСИЛ по данным магнитно-резонансной томографии (МРТ) у пациентов из российской популяции.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 16 человек, у которых диагноз ЦАДАСИЛ подозревался на основании клинических данных и МРТ-картины и подтвержден молекулярно-генетическим исследованием гена NOTCH3. Собраны и проанализированыданные МРТ головного мозга 16 пациентов и МРТ спинного мозга 5 пациентов. У 2 пациентов выполнен анализ цереброспинальной жидкости (ЦСЖ) для дифференциальной диагностики патологического процесса.</p></sec><sec><title>Результаты</title><p>Результаты. Патологические участки гиперинтенсивного сигнала в белом веществе головного мозга выявлены в 100% случаев. Поражение передних отделов височных долей и наружных капсул зарегистрировано у 62 и 68% пациентов соответственно. Множественные лакунарные инфаркты являются характерным признаком ЦАДАСИЛ. Микрокровоизлияния выявлены у 27% пациентов. Атрофические изменения наблюдались в 56% случаев. У 2 пациентов выявлено поражение мозолистого тела и шейного отдела спинного мозга.</p></sec><sec><title>Заключение</title><p>Заключение. Выявление на МРТ гиперинтенсивности белого вещества в лобных и теменных долях, а также множественных лакунарных инфарктов позволяет отобрать группу пациентов с подозрением на ЦАДАСИЛ для дальнейшего молекулярно-генетического тестирования на предмет мутации гена NOTCH3.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is common variant of small vessel disease. Determination of neuroimaging markers have already contributed at an early stage to the more timely diagnosis of the CADASIL.</p></sec><sec><title>Objective</title><p>Objective. The aim of the study was to determine probable CADASIL magnetic resonance imaging (MRI) biomarkers for patients from the Russian population.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 16 patients. Diagnosis of CADASIL was suspected based on clinical data and an MRI and confi rmed by a molecular genetic study of the NOTCH3 gene. Brain MRI data from 16 patients and spinal cord MRI data from 5 patients were collected and analyzed. Cerebrospinal fl uid (CSF) analysis was performed for diff erential diagnosis in 2 subjects.</p></sec><sec><title>Results</title><p>Results. White matter hyperintensity was observed in 100% of cases. The involvement of the anterior temporal lobes and external capsules was revealed in 62 and 68% of subjects respectively. Multiple lacunar infarcts are characteristic for CADASIL. Microbleeds were detected in 27% of patients. Brain atrophy was observed in 56% of cases. Lesions in corpus callosum and cervical spinal cord were found in 2 patients.</p></sec><sec><title>Conclusion</title><p>Conclusion.
